How To Renew A Lost and Expired Passport



Renewing a lost and expired passport can be a daunting task. Apart from the usual IDs and documents required upon renewal, you will also be asked to present other requirements such as an affidavit of loss. The Department of Foreign Affairs (DFA) provides a straightforward and convenient process for Filipinos to renew passports, even in situations where the passport has been lost or misplaced. In today's blog, we will guide you through the steps to renew a Philippine passport that has expired and is missing and could not be found.

The first thing you need to know when renewing a lost and expired passport is that your application for renewal will be treated as a new application by the DFA. Therefore, you will be required to present documents that are only commonly required of first-time applicants, such as PSA certificates. Requirements when renewing a lost and expired passport:

1. Confirmed online appointment at your preferred DFA branch

2. Personal appearance at your chosen DFA branch on the time and date of your appointment.

3. Accomplished application form.

4. Original and photocopy of PSA authenticated birth certificate on Security Paper. 

  • Married females who are using their spouse's last name must also present original and submit the photocopy of PSA authenticated marriage certificate on Security Paper or Report of Marriage.
  • Local Civil Registrar copy is required if the PSA birth certificate is not clear or cannot be read.
  • If photocopy of the applicant's last issued passport is available, the Birth Certificate will not be required.
  • If female applicant opts to retain her maiden name, a marriage certificate is not required. 
5. Any of the following acceptable IDs, with one photocopy for submission:
  • Philippine Identification (PhilID)/ePhilID
  • Social Security System card
  • Government Service Insurance System (GSIS) card
  • Unified Multi-purpose Identification (UMID) card
  • Land Transportation Office (LTO) Driver's License. Student Permit may be accepted if in card format.
  • Professional Regulatory Commission (PRC) ID
  • Overseas Workers Welfare Administration (OWWA) E-card
  • Commission on Elections (COMELEC) Voter's ID or Voter's Certificate issued from COMELEC main office in Intramuros, Manila
  • Philippine National Police (PNP) Permit to Carry Firearms Outside Residence 
  • Senior Citizen ID
  • Airman License (issued August 2016 onwards)
  • Philippine Postal ID (issued November 2016 onwards)
  • Seafarer's Record Book (SRB) or Seafarer's Identity Document (SID) must be issued February 2020 onwards and issued by the Maritime Industry Authority (MARINA)
  • School ID if applicable
And then again, the DFA may require other supporting documents not listed here, as they see fit, so just be ready to present other IDs and documents.
